Right, this PDCA cycle thingamajig sounds vaguely familiar, doesn't it? So, the crux of the matter involves figuring out how to improve things. Interestingly, its all about a continuous loop of planning, doing, checking, and then acting. I once was tangled in project planning where we implemented a new software system; it was all smooth sailing until the feedback pointed out glaring issues. In essence, we failed in the Check phase!
